Performance Enhancing Drug Survey

You have reached the web site to take a survey on the use of anabolic androgenic steroids in high school athletes that is being conducted by researchers from the David Geffen School of Medicine at UCLA. By clicking on the link below, you will be directed to an on-line questionnaire that will take approximately 5 minutes of your time. You will need the password that was provided to you at your school. Each individual may only complete the survey ONE time.

There is no penalty for not participating in the study and your participation in this study will not affect your athletic participation at your school. All questionnaires are totally anonymous and individual results will not be given to your principal, athletic director, coach, or anyone else at your school.
